By submitting this form, I am requesting that my child be released from school for up to one hour and 30 minutes one day per week throughout the school year for the purpose of attending the Christian Weekday Religious Education (CWRE) program. I believe that my child will benefit from the lessons that will be taught during this time and I understand and consent to the following:

I grant my permission for my child to be transported either by van or by walking to and from the CWRE sites where the program will be held. See the addresses of the sites below.
I understand that specials such as art, music, gym, etc., or academic instruction will continue during the release time.
I also understand that my child is responsible for making up any school work that they may miss during this time.
If my child’s grades are impacted in a negative way, as a result of participating in the release time, I will consider withdrawing my child if I feel it is necessary in order for my child to maintain grades.
My child is expected to follow the rules given by the CWRE leaders, teachers, & van drivers. If discipline issues arise that cannot be resolved I understand that my child may be asked to withdraw from the release program.
I understand that If my child is offered an aid, due to 504 accommodations or an IEP, that the aid will not be able to attend the release time with my child.
I understand that once my child is released from School to attend CWRE that all responsibility, liability & care of my child is transferred to the CWRE Team until they are returned back to school.

Site Addresses for the CWRE Program:

Central, Van Buren, Brentwood and Clarks Creek will attend: Friends Church, 105 S. East St., Plainfield IN (Central & Van Buren Students will walk to and from the site, Brentwood and Clarks Creek students will ride vans.)

Guilford will attend and be transported by vans to: Center Community Church, 5938 S. Co. Rd. 700 E., Plainfield IN
